Funding from Human Services for Family Support for Children with Disabilities (FSCD)

• I’m for Kids Team provides Specialized Services to families with existing FSCD Agreements and priority is given to those children accessing PUF via I’m for Kids Team.
• It is our belief that ONE TEAM can best provide coordinated services for two separate funding sources for one child/ family.
• I’m for Kids Team was one of the first to participate in the PUF/FSCD pilot project and are able to bring our years of experience to this service delivery model.
• Our FSCD Coordinator has many years of experience and oversees all programming decisions and implementation.
• Our children receive coordinated services through the strong team of professionals (Speech Language Pathologist, Occupational Therapist, Physical Therapist, Psychologist, Educational Behaviour Specialist, Family Support Assistant ) that are assigned to each child’s FSCD team.
